Physical Form
A physical form must be filled for your child to participate in football or cheerleading.
Work Bond
Each family is required to work 3 work duties to help with our volunteer organization. To receive proper credit for completion of your obligation, YOU MUST SIGN IN AT THE SNACK STAND to get credit for you WORK DUTY. Thank you for your dedicated volunteer service!’

Equipment Bond
Each family is required to give 1 Equipment bond check. Your equipment bond check will only be cashed if all equipment is not handed in at the end of the season.
